1: Create a database in mysql named 'logs'
2: Create the table inside the database named 'syshistory_minute' with the 'create_table.sql' script
3: Edit the mysql connect parameters in "syshistory_crontab"
4: Edit the mysql connect parameters in "config.inc.php"
5: Place the html directory somewhere in your webroot
6: Place the "syshistory_crontab" in /usr/local/bin and use this in your crontab like:
"* * * * * /usr/local/bin/syshistory_crontab >/dev/null 2>&1"
